If you are new to the SMPA electrical service area or are requesting service for new construction, please fill out our Electrical Service/Membership Application
If construction is involved, download our Service Engineering Request Form. * The Constuction Handbook is a printable version of the entire booklet and is intended to be folded and stapled. You can also view our Service Connection and Line Extension Policy
*All forms may be mailed or faxed to your local SMPA office.
